Job Description

Uses the nursing process to provide patient care. Identifies and utilizes resources for complex patients care situations. Supervises other members of the clinical nursing team. Demonstrates standards of professional practice.

1. Applies nursing process to the development, implementation and evaluation of nursing care.

EP=Utilizes critical thinking skills independently. Anticipates subtle changes in patients status and reacts accordingly. Recognized by peers as an expert in caring for department specific populations of patients with multiple diagnoses and unexpected needs.

SP=Recognizes changes in patients conditions and revises plan of care appropriately. Identifies variances in the plan of care and makes adjustments as needed. Identifies and addresses educational needs to patients/significant others.

ND=Requires prompting, reminders and direction to meet patient/department needs resulting in follow-up. Documentation is incomplete. Does not consistently follow plan of care. Fails to develop and implement individualized plan of care. Inconsistent rounding.

NR=Completing orientation or in role less than 6 months at the end of the performance period. Making expected progress toward performing all items outlined in the MWA

2.Supervises other members of the team

EP=Demonstrates leadership abilities as a preceptor/mentor and is recognized as a resource/facilitator for precepting, mentoring, delegating, and team building.

SP=Delegates to support staff with a collaborative spirit. Provides constructive feedback. Supervises select personnel in performance. Optimizes resources to positively affect patient care.

ND=Lacks appropriate supervisory and delegation skills, Follow up is not consistent.

NR= Completing orientation or in role less than 6 months at the end of the performance period. Making expected progress toward performing all items outlined in the MWA

3. Supports an environment of evidence based practice/research/quality improvement

EP=Participates in the design and implementation of multi-disciplinary quality improvement activities. Participates in nursing research activities. Demonstrates evidence of the Iowa model and participates in the departmental clinical practice change projects.

SP=Recognizes opportunities to improve clinical practice and shares them with appropriate Shared Governance Representative. Participates in unit/department based quality improvement activities. Coaches and mentors others related to shared governance principles.

ND=Does not participate or demonstrate knowledge of QI activities. Does not understand and demonstrate knowledge of evidence-based practice. Does not demonstrate knowledge of Shared Governance accountabilities and principles

4 Establishes a caring and therapeutic nurse/patient relationship

EP=Participates in the design and implementation of models of caring

SP=Proactively analyzes and responds to patient needs/requests. Consistently receives positive feedback from patient, families and significant others.

ND=Does not always demonstrate sensitivity towards individual patient/significant other needs related to age, culture, other variables. Does not interact in a non-judgmental, non-discriminating manner. Does not demonstrate the ability to advocate for the patient/significant other. Does not always demonstrate a caring spirit to obtain optimal patient outcomes.

5 Demonstrates effective conflict resolution/negotiation skills

EP=Facilitates building and preserving collaborative relationships. Participates in Service Recovery.

SP=Demonstrates the ability to problem solve and assist with resolution of conflicts.

ND=Does not bring forth issues related to conflict resolution/problem solving/confrontation. Does not seek constructive feedback and use it to make positive changes.

Qualifications

1 year of nursing

1.Stand and/or walks for majority of shift.

2.Bends and/or stoops frequently.

3.Lifts thirty pounds or more four times per hour.

4.Sufficiently mobile to push stretchers, wheelchairs, carts and equipment.

5.Communicates via telephone and other automation devices.
6.Visualizes computer screens, medical record forms and equipment screens.

7.Lifts and positions patients of all weights and heights using assistive devices and personnel as appropriate.

8.Hears blood pressures, IV alarms, call bells, monitor alarms, and other audible alarms used for patient safety.

9.Physical job demands may be subject to possible modifications to reasonably accommodate individuals with disabilities.
