Vidant Medical Center is a fully accredited, 900+ bed regional referral facility encompassing a Level-1 Trauma Center and extensive inpatient and outpatient services including intensive and intermediate care, pediatrics, bariatrics, obstetrics, and gynecology, surgery, pain management and psychiatry.
Works collaboratively and maintains active communication with physicians, nursing and other members of the health care team to effect timely, appropriate patient management.
Utilizes clinical skills and assessment findings to lead interdisciplinary team in plan of care development and implementation.
Identifies delays or alterations in the plan of care and addresses/resolves system (internal and external) problems impeding diagnostic or treatment process to ensure delivery of care in the most appropriate setting.
Meets directly with the patient/family to perform initial discharge planning assessments and coordinates with the Inpatient Social Work Case Manager to proactively address complex psychosocial issues and barriers to efficient discharge planning.
Makes the necessary arrangements to facilitate and coordinate support services for discharging patients to significantly improve outcomes, through safe, sustainable discharge plans and appropriate follow up.
Understands and implements the Case Management role in Code Yellow Emergency Operations Plan and Capacity Management Plan, as outlined in their specific policies and procedures.
Assesses for and uses appropriate interventions regarding traumas, domestic violence, sexual assault, abuse/neglect, guardianship and other complex psychosocial or medical issues.
Assesses for the appropriate utilization of resources, avoidable admissions and/or high-risk discharge planning/psychosocial issues.
Utilizes the nursing process to assess, plan, implement and evaluate the discharge needs for patients in assigned caseload.
Demonstrates proactive, coordinated and effective discharge planning skills.
Negotiates with patient/family, physicians and other interdisciplinary team members to identify and prioritize issues and coordinates a comprehensive plan of care and subsequent discharge plan.
Collaborates and communicates with interdisciplinary team members in all phases of the discharge planning process to facilitate plan of care, including external and internal resources, such as Inpatient Social Work Case Manager, Utilization Review Nurse, Case Management Supervisor, Home Health or DSS liaisons, etc. for high risk discharge planning/psychosocial issues.
Documents the relevant discharge planning information in the electronic medical record according to department guidelines.
Identifies issues and strategies to modify and/or enhance the case management program.
Assists in the collection of data and reporting of financial indicators and utilizes data, such as case mix, LOS, cost per case, resource utilization, denial and/or delay days, to drive problem identification and resolution of clinical inefficiencies.
Proactively changes practice behavior as a result of data analysis.
Advances clinical expertise and promotes professional growth through attendance at educational opportunities, conferences, reading journals and meeting requirements for mandatory continuing education.
Accountable for current knowledge of case management responsibilities, including regulations and payer information.
Participates in professional organizations and maintains a professional network among colleagues.
Current RN licensure in the State of NC or hold a class P Temporary License as a Registered Nurse in NC
BSN in nursing from an accredited college or university preferred with 4 years clinical experience
ADN nursing degree with 5-7 years strong clinical experience
Evidence of leadership experience, e.g. charge nurse or primary nurse
Clinical experience in critical care areas preferred
Evidence of at least one year of previous experience with case management, discharge planning or utilization management preferred
Salary Range $23.40 - $37.44.
