The Advanced Practitioner provides comprehensive care to patients requiring both acute and chronic medical management. Collects and assesses patient data through history gathering interviews and focused physical examinations to formulate appropriate patient centered plan of care. Implements a plan of care utilizing appropriate diagnostic studies and treatments, conducts follow-up examinations to monitor and evaluate the plan of care. Provides patient/family education and/or counseling and evaluates the outcomes of care using the clinical skills and judgments inherent in nurse practitioner education and practice. Works in collaboration with physicians and other health care team members, consulting and referring as appropriate. Acts as a consultant to colleagues and health care team members in the area(s) of clinical expertise; serves as a resource for the staff and other health care providers. Demonstrates ability to function independently in the outpatient setting. Maintains and updates clinical knowledge and actively seeks opportunities to broaden clinical expertise. Participates as a member of the health care team to develop policies, clinical protocols and evaluation tools. Adheres to system and department compliance policies, and any applicable laws and regulations.
Valid Massachusetts Physician Assistant licensure or Nurse Practitioner licensurewith certification as an Adult or Family Nurse Practitioner, thorough familiarity with ambulatory medical care. This position includes longitudinal care and urgent care of typical internal medical practice with acute and chronic care of outpatients.
