The ECC Case Manager is responsible for facilitating the patient's ECC hospital stay following provider evaluation until discharge, transfer and/or inpatient/observation admission promoting the safe and effective transition of care. The case manager coordinates and negotiates with physicians, nurses, discharge planning, transitional care and other healthcare team members to expedite medically appropriate cost-effective care. The case manager applies clinical expertise and medical appropriate criteria to resource utilization, discharge planning and transition care as needed. The case manager will advise the healthcare team and provide leadership as needed. Work schedule may vary dependent upon ECC and other departmental needs. The ECC case manager reports to the Director of Case Management.
Current RN license in the State of NC or hold a class P Temporary License as a RN in North Carolina. BSN in nursing from an accredited college or university preferred however ADN or diploma program acceptable. Evidence of at least 3 years of clinical competence in providing direct nursing services to acutely and/or chronically ill patients in inpatient and/or outpatient settings. Evidence of at least 1 year of previous experience as anurse case manager or discharge planner with clients in the clinical specialty area desirable.
